All JLM Members: A new survey has been created to be completed when you pay your dues, either by check or online. Please click on the link below to download it in a Word document format.

This information is being gathered for the purpose of maintaining demographic data of JLM. We can assure you that your answers will be kept confidential and will be used to determine the growing needs of JLM and for no other purpose.

Please print it out and return it to JLM Headquarters with your dues payment, or send by email to Aniella Gonzalez. All responses are strictly anonymous.
